All players must submit a Medical Form before participating in any NH Fastbreak practices or games. A completed form should be given to the Head Coach or the Team Manager as soon as possible, and are usually collected at the first practice. You can download the form here (Medical Form). This form is used in the event of an emergency only. If a player is injured during a practice or game, and the parent or guardian cannot be reached, this form will help us in providing the necessary information to the emergency health care provider. It is the responsibility of the Head Coach or Team Manager to have copies of these forms with them at all times during AAU-sponsored events.
Participation in practices is mandatory for all players. If a player is going to be late or absent from a practice or game, they should contact the Head Coach or Team Manager as soon as possible to inform them. It is not fair to the other players to hold up a practice or game for another player that shows up late or never shows. Players that abide by this rule will not be penalized in any way (as long as the lateness or tardiness is not excessive). Players that fail to abide by this rule may have their play time affected at the discretion of the Head Coach. No official practices will be held during the April School Vacation week when most local schools are not in session. Coaches may hold optional practices during this time at a location arranged by them. Also, the NH Fastbreak club may hold an optional camp or clinic during this time. The cost of these optional activities is not included in the team costs for the season, and must be paid for separately by the participants.
